Hi,
If you're abroad, contact your local STA Travel agency (http://www.statravel.com.
If you're in Russia, there's a local branch of STA Travel - "STAR Travel", you can get it there - $7 US, made in 5 minutes, your student ID, passport, and 1 photo is needed.
