I am applying for a multiple-entry business visa. The visa rules state that for stays longer than 3 months an HIV Test Certificate is required.

Do you know what this test certificate should look like? I had the test done recently at a local medical center. When I asked the doctor about it, he gave me a print out with the test results. Should this test certificate be signed and/or stamped or would a print out from the medical center's computer system suffice?

Well it will depend on which embassy/consulate you apply to as to how picky they are but 'certificate' suggests that it should at least have you name and date of birth on - speak to the embassy/consulate and see what they say. To be certificated you normally need to pay for the test as opposed to getting a free test as given by GUM clinics.

I went to my local health department....told them what I needed and why (it was free, btw). They supplied me with "an official document" which satisfied the powers that be...or were.
I don't really recall what it looked like.....or if it was "stamped."
It was on "state" stationary......and was signed.
What country are you applying from?
