Hi,
1.In that form you are asked to type the first six cities you will be visiting but you cannot type that many as it doesn't fit.
2.I do not have a fax machine.
3. I do not have a middle name.

I do assume if you request the original papers straight away to be sent by post that will be done? would that include an extra charge?

1. Then type as many as fits in the form -- it's just a check to see through where you will enter Russia and through where you leave. So put the entry/exit points the first and the last and whatever fits in the middle.

2. You will receive your visa support by e-mail (as an attachment).

3. Then do not enter anything there.

If you require the documents to be sent to you by post, it will incur extra charge which will basically be the same as the post charges. So if you prefer the slow Russian post (takes about 2 weeks to Europe, a month to USA) it's going to cost about $3, if you use courier service it'll cost $30.

You don't need originals in most cases though.
