As a US Citizen from California, I am planning to visit Russia as a tourist for 2 weeks.

First question of item 33 is:
Have you ever been arrested or convicted for any offence?

Do I need to say Yes, for a drunk driving (Driving Under the Influence - DUI) related arrest 13 years ago!?

I am reluctant to check Yes, for the fear that it may result in a delay or worst yet, a denial of visa!? Since the question does not ask for the type of offence, I am afraid that the consulate may interpret it as the worst possible offence and ask me to appear in person! And I am not likely travel 500 miles to do so!

By the way, how important are the dates, addresses, phone & fax numbers of the last two employers (item 27) for I don't remember this information for the two employers I worked for 15 years ago in a city 130 miles from where I live now!?

Just answer no. They don't have access to this information. You just might go to hell for lying, hahaha.
I always leave blank the previous employer list.
There is another funny one : the list of countries and cities visited in the last 10 years. I put 3, I think that shows I'm answering seriously. Don't mention your visit to countries openly hostile to Russia. USA is bad enough, don't add (Eastern) Georgia and Estonia, for example.

This is OK; just be sure to list any countries that your passport will indicate that you have visited.
